I'm both intensely curious as to how this could have possibly happened and pretty sure I don't want to know.

(Pleural fluid is the fluid between the serous membranes around the outside of your lungs. Key word: "outside", so the patient somehow getting semen in their lungs only explains how it got to the general area. Though, I guess that might have something to do with why they're in the hospital in the first place)
